Please fill out the Cable TV service form at the Telecommunications Department, Smith 207. You can print the form here and hand it in. Cable will be activated on a first-come, first-served basis.
Please understand that you are responsible for the payment of cable TV service. If payment is not received by the 15th of the month, service will be disconnected. A $10.00 reconnection fee must be paid before service is reinstated. If a bill is not received by the 10th of the month you must call x4000 for a duplicate bill.
Unauthorized cable television connection is a federal offense under the Cable Communications Policy Act of 1984, which went into effect on Dec. 29, 1984. The Federal Law clearly established a cable operater's right to leagal action against thieves and monetary awards for damages and lost profits. Both Ohio and Federal Law prohibit theft of service and impose fines of up to 6 months in prison, $1,000, or both.
